At Kaiser Permanente’s San Diego Preventative Medicine clinic, the 10-page Health Appraisal Questionnaire (HAQ) is an important tool in providing high quality patient care. Since every patient who visits the clinic completes an HAQ, the department processes more than 500,000 pages of detailed medical history data each year. The system implemented by Hershey uses high speed scanners from Fujitsu and Cardiff TeleForm software, which offers powerful data recognition capabilities from scanned documents. |
